Application check list for applying to universities

  • Application form print out( not needed if you apply online)
  • Department supplemental forms (GA/TA/RA forms) (needed if you plan to apply for those posts, they are available in the univ website)
  • Immunization forms( they are needed only after you get Visa and visit the univ. not needed for the purpose of application)
  • Application Confirmation page
  • Payment page 
  • OFFICIAL TRANSCRIPTS FROM ALL  THE EDUCATIONAL INSTITUTIONS ATTENDED  in sealed envelopes.
  • 10 & 12TH ( Attested by respective 10th and Inter College Principals OR Engg Principal OR any Gazzeted officer  in Sealed Envelope)
  • RECOMMENDATION LETTERS ( From three Professors, Signed  in Sealed Envelopes)
  • STATEMENT OF PURPOSE/  PERSONAL STATEMENT
  • RESUME/ CV
  • GRE/TOEFL TEST SCORES (Must be submitted directly from ETS. Xerox copies can be enclosed but not mandatory)
  • BANK STATEMENT
  • AFFIDAVIT OF SUPPORT
  • REFERENCE LETTER FROM INDUSTRY EXPERTS/ ORGANISATION (if available)
  • PHOTOGRAPHS  ( usually not needed but you can send 2 IN NUMBER)
  • PROJECT & PAPER PRESENTATION CERTIFICATES ( XEROX copies – 1 Set enclosed in an envelope)
  • WORK EXPERIENCE LETTER
  • IMMIGRATION DOCUMENTS – XEROX of Passport
 These is the most common version of checklist needed to apply to Universities in USA. But it may wary from university to university and few documents may/may not be needed.
